The Patient Advocate Foundation (PAF) Co-Pay Relief Program (CPR) currently provides direct financial support to insured patients, including Medicare Part D beneficiaries, who must financially and medically qualify to access pharmaceutical co-payment assistance. The program offers personal service to all patients through the use of call counselors; personally guiding patients through the enrollment process.
Are You Eligible?
The Patient Advocate Foundation (PAF) Co-Pay Relief Program (CPR) currently assists insured patients who are financially and medically qualified and are being treated for breast, lung, prostate, kidney, colon, pancreatic, head/neck cancers, malignant brain tumor, sarcoma, diabetes, multiple myeloma, myelodysplastic syndrome (and other pre-leukemia diseases), osteoporosis, pain, hepatitis C, rheumatoid arthritis, selected autoimmune disorders and CIA/CIN.
For Providers
Currently the PAF Co-Pay Relief Program offers a dedicated, secured web site for medical providers to enroll electronically for the CPR Program on behalf of their patients. Should you wish to access this provider resource, please contact the CPR Program toll free at (866) 512-3861 or (757) 952-0118 to obtain information on how to register. You will receive conformation after registering via e-mail with your username and password for the site allowing for future utilization.
Please Note that the provider tax ID number is required to complete the enrollment process.

We are pleased to now offer a dedicated, secured web based application for patients to enroll electronically for the Co-Pay Relief Program (CPR). Please be advised that only one application per patient will be accepted. The patient’s social security number is required to complete the application process. Should you wish to access this patient resource, please click on the link below.
